I/We, the undersigned Adopter/s, understand and agree to the following terms of this contract in order to adopt the pet, further described with in this document. I/We understand that non-compliance with the terms of this agreement gives Underdog Salvation the right to reclaim this pet without refund of the adoption fee or other compensation. The contract shall remain in effect for the life of the pet, or until the return of the pet to Underdog Salvation.

Please read this contract carefully as it is a legally binding document. All fields are mandatory. If you cannot fill out a section or disagree with the contents of any portion, please contact Underdog Salvation to discuss. Thank you for adopting!

Terms & Conditions*
- I am adopting the pet described below through Underdog Salvation. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not allow the pet to be used for purposes of vivisection or experimentation.
- I will provide proper and sufficient, food, water, shelter and kind treatment for the pet at all times. I further agree the dog will not be chained at any time as a means of restraint or be a primarily outside dog.
- I agree not to alter in any way the physical appearance of the animal being adopted unless deemed medically necessary by a licensed veterinarian. This includes declawing, cropping of ears, and docking of tails.
- I am financially able to provide all routine and emergency care for this pet for his/her lifetime. This includes but is not limited to food, boarding (if necessary), regular vet care, heartworm preventative, and flea/tick preventative.
- I agree to immediately notify Underdog Salvation if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
- I understand that Underdog Salvation makes no guarantees or warranties regarding the health or temperament of this pet. Underdog Salvation, it’s heirs or representatives, shall not be held liable for the behavior of this animal or any damages it may cause.
- I agree to take full responsibility of any medical bills after the adoption (we recommend your new pet be seen by a veterinarian within the first two weeks of adoption). Medical bills will not be reimbursed by Underdog Salvation at any time.
- I certify that all statements made by me in this adoption agreement are true and correct. I agree that Underdog Salvation has the right to confiscate the above described animal in the event that any statements made by me are found to be false and/or my check for the adoption fee is returned for insufficient funds.
- Underdog Salvation has provided me (or will provide me upon arrival of the dog) with the following: Adoption Contract + Health Records
